Chronic Disease Management Outreach Program (CDMOP)
The Chronic Disease Management Outreach Program (CDMOP) provides additional evidence-based diabetes and chronic disease self-management classes to the community, including classes and materials in English, Marshallese, and Spanish.
CDMOP will also work to reduce hospital readmissions through the work of the Population Health Coordinator, who will teach classes and work with both INTEGRIS Bass Baptist Health Center and St. Mary’s Regional Medical Center to identify patients in need of chronic disease management information and resource coordination.
The Community Health Workers provides translation, navigation assistance, resource coordination, teach classes, and other, as-needed activities for their respective communities.
Additionally, the CDMOP provides Academic Detailing and Practice Facilitation.
